Minimally invasive spine surgery, also known as MISS, has no specific meaning or definition.

Scoliosis - If your curve is greater than 45 to 50 degrees, surgery would likely be recommended, however, several factors may influence the decision. In your Scoliosis surgery, spinal fusion is done, where the curved vertebrae are fused together so that they heal into solid bone. This will prevent the curve from getting worse.

Spinal fusion is surgery to join two or more vertebrae into one single structure. The goal is to stop movement between the two bones and prevent back pain. Once they're fused, they no longer move like they used to. This keeps you from stretching nearby nerves, ligaments, and muscles that may have caused discomfort.

Specialized Treatments

Minimal Invasive Spine Surgeries

Minimally invasive spine surgery (MISS) used advanced technology and innovative techniques to treat back and neck pain caused by a variety of spinal disorders. Minimally invasive spine surgery minimizes soft tissue damage, may be less risky and is a less invasive option compared to traditional open spine surgery. In general, Minimally invasive spine surgery decreases the hospital stay by one-half. Typical endoscopic discectomy surgeries are performed on the same day, and the patient goes home shortly after the surgery on the same day. However, after lumbar fusion surgery, the patient goes home in 2 to 3 days.

Spinal Tumor

If you have a slow-growing metastatic spinal tumor, or only one spinal tumor and have neurological deficit such as weakness, loss of muscle control and/or significant spinal instability then, you may require spinal tumor surgery along with stabilization of vertebrae with a curative goal. You may undergo an extensive surgical procedure and stabilization method shall depend upon the tumor type, the extent of bone destruction, general condition and anticipated cancer treatment.

The fusion shall involve titanium rods, screws & cages with the assistance of spinal navigation. Surgery would last for 4-8 hrs depending upon the tumor & the extent of stabilization procedure involved.

Spinal Injections

Spinal injections are a pain management treatment that is recommended to relieve or reduce back pain. They can be either used to diagnose the source of back, leg, neck, arm pain or used as a treatment to relieve pain.

Spinal injections are performed under X-ray guidance called fluoroscopy. Fluoroscopy makes sure that the medication is placed correctly. Before the medication is given, a liquid contrast is injected. Until the correct flow is obtained, an additional dye is used. This may also include repositioning of the needle. The injection is then given.

Vertebroplasty & Kyphoplasty

Vertebroplasty and kyphoplasty are both minimally invasive surgical procedures for treating osteoporotic fractures where a cement-like material is injected directly into the fractured bone. This stabilizes the fracture and provides immediate pain relief in many cases.

Kyphoplasty includes an additional step. Prior to injecting the cement-like material, a special balloon is inserted and gently inflated inside the fractured vertebrae. The goal of this step is to restore height to the bone thus reducing deformity of the spine. Most patients return to their normal daily activities after either procedure. Kyphoplasty is a minimally invasive surgery used to treat a spinal compression fracture. The goals of kyphoplasty are to reduce pain from the fracture, stabilize the vertebra, and restore the vertebra back to its normal height.

In the procedure, the patient lies face down on the operating table. The surgeon makes a small, half-inch incision over the affected area. Using X-ray guidance, the surgeon inserts a narrow tube through the pedicle into one side of the fractured vertebra. In balloon kyphoplasty, a balloon tamp is then inserted through the tube and into the fractured vertebra. Once inside the vertebra, the balloon tamp is inflated to create an open cavity inside the bone and to restore height to the collapsed vertebra. The surgeon then deflates and removes the balloon tamp from the vertebra, leaving the new bone cavity behind. A pasty compound called PMMA, a type of bone cement is injected into the cavity until it is full. The cement hardens quickly, creating an internal cast inside of the fractured vertebra.

The surgeon then closes the incision. The patient stays on the operating table while the cement hardens, which usually takes about 5 minutes. Most patients can go home the same day as the procedure.

Pain Management

During any spine surgery, anesthesia is provided so that patient does not feel any pain or be uncomfortable. After the surgery, pain control becomes necessary to give patients the chance to go home after an outpatient spine surgery. Pain is managed by reducing the size of the exposure while performing the surgery. The deeper muscles are should also be infused with anesthetics. Pain management is also done with the combination of narcotics medication, muscle relaxers, and inflammation medications. Ice and cool therapies can also reduce the postoperative pain and swelling.
